PHMS PTSA's Star Search 2012

Auditions for Star Search 2012 will be held in the evening on March 13.

Have your parent/guardian go to the PHMS PTSA website (www.phmsptsa.org) and fill out and submit the audition registration form. (The Public Library has free Internet access if you need it.)
Have your parent/guardian fill out and sign the permission slip below and bring it to audition night.
If selected for the show, you must be able to attend dress rehearsal on April 24 and the show on April 27.

ONLY SUBMIT THE ONLINE FORM IF YOU CAN AUDITION ON MARCH 13.

Singing! Dancing! Pop Instrumental! Gymnastics! Groups! Rock Bands!

~~Only 40 acts will be able to audition on a first-come basis so don’t delay.~~

Songs will not be duplicated so get your selection in early!

Audition Rules:

DO: come at your assigned time. (You will be contacted.)	DON’T: bring friends. (Parents must stay.)
DO: bring only a CD of your music.	DON’T: bring other formats.
DO: use karaoke or instrumental music only.	DON’T: bring tracks with the artist singing.
DO: choose appropriate music and dress.	DON’T: have explicit lyrics.

Parent signature is required. It means you will follow these rules. Bring this signed form on March 13!

Star Search 2012 Permission Slip

My child ____________________________________________ has permission to audition for, and, if selected, attend rehearsals and perform in the 2012 Star Search Talent Show, and to be photographed and videotaped for this event. He/she will follow the audition rules, respect the decisions of the coordinators, and adhere to expected school behavior at all times.
